Tele-Medicine provides greater access to care to decrease travel, eliminate waiting times, and crises situations managed timely without excessive delay, and for overall enhancing patient experience and outcomes. 

At Perspective, we understand that proper technology equipment and connective devices are needed for this service and will work with those who do not have access, at no-charge to you.

Wound Care Services

Following on to gathering of a detailed patient’s past medical history, intrinsic and extrinsic factors as well as the assessment of the wound bed. The clinician must focus on wound bed preparation (WBP). Wound bed preparation is recognized as the management of wounds in order to accelerate endogenous healing or to facilitate the effectiveness of other clinical therapeutic measures. Both acute and chronic wound management involves holistic assessment and on-going evaluations of the wound bed in order to enable positive patient outcomes. Timely wound assessments and reviews encourage effective clinical decision making and as a result influences the best clinical action to support wound bed preparation. Effective wound bed preparation is an essential, ever evolving element in chronic wound management. It helps to identify and eliminate local barriers which in turn may facilitate wound healing and provide a more effective means of wound management by:

  • Promoting an understanding of the barriers to healing.
  • Providing a systematic approach.
  • Enhancing the effects of advanced therapies.

 

The wound bed preparation can be summarized using the TIME framework which can be diagrammatically presented as in Figure 2. The framework offers clinicians a considered approach to selecting wound interventions by systematically going through each of the following components:

  • T= Tissue non-viable or deficient
  • I= Infection or inflammation
  • M= Moisture imbalance
  • E= Edges of wound – non-advancing or undermining.

Perspective BPS ensures a seamless referral process for both Resident and Primary Care Physician office. 

  • Secure E-Fax or Practice Fusion EMR Integration
  • Physician order for Specialist Consult
  • Most recent Progress Notes, Laboratory and Diagnostic Reports
  • Current Medication List and Pharmacy information
  • Demographics and Insurance information 
  • Additional pertinent documentation

Our intake specialist will review submitted documents within 24-48 hours and coordinate with patient, family member or facility representative to schedule an appointment.
